WHAT IS FISSURE?

If you are experiencing an anal fissure, it likely causes a lot of pain, which can be very distressing. Anal fissures are something that most people have to deal with at some point in their lives, so knowing the symptoms of it is the best way to decide if you should see your doctor.

An anal fissure is a painful condition that occurs around the opening of your anus. The nerve endings inside your anus or rectum are exposed to pressure and irritation when stools get pressed against it. As a result, you may experience severe pain, bleeding from the anus, fever, rectal swelling, and more.

What happens during an anal fissure?

It is possible to have anal fissures without pain or discomfort, it is typically caused when the muscle that runs in front of the anus becomes too short. The problem is, when there are fissures in your intestines, they will naturally cause some discomfort along with bleeding and itching. A lot of people don't know this, however, so at first, it may seem like your symptoms are entirely in your head until the doctor has thoroughly examined you.

How can a fissure be repaired?

Anal fissures often disappear within a few weeks with the right at-home care. For instance, increase your fiber and fluid intake to keep your stools soft. Take warm water soak for 10 to 20 minutes multiple times every day, especially right after a bowel movement. The healing process might be sped up by relaxing the sphincter.

What are the surgical methods?

  1. Laser Sphincterotomy - It is a medical procedure used for treating chronic anal fissures. During the course of the treatment, the surgeon employs a laser to create an incision in the tight anal sphincter muscles. This incision helps to relax the muscles, thereby allowing for the fissure to heal in a natural manner. This method of treatment has become increasingly popular in recent years due to its non-invasive nature and its ability to provide patients with quick and long-lasting relief from their symptoms.
  2. Botox injection - It is another treatment option that is gaining popularity among those suffering from chronic anal fissures. This procedure involves the injection of a weak poison (toxin) into the anus to temporarily paralyze the muscles of the anal sphincter. This can help to reduce pain and itching and encourage healing of the fissure. Additionally, it can take effect more quickly than LASER treatment, providing near-instant relief from symptoms.
  3. PLIS (Partial Lateral Internal Sphincterotomy) - It is a surgical procedure for treating anal fissures. It is often performed under local anesthesia and sedation and involves making a small incision in the external sphincter muscle in order to reduce the spasm and tension that can contribute to the formation of anal fissures. The procedure is generally successful in treating such fissures, with a reported success rate of 80-90%. Furthermore, it has the benefit of being both minimally invasive and relatively low risk.
  4. Lord’s Dilation - When performing the lord's dilatation (or lord's), which requires widening the anus that is already open, a speculum is placed into a patient's rectum at different angles. This procedure is carried out to give the patient more room and to lessen suffering from hemorrhoids or constipation.

Frequently Asked Questions (FAQ's)

Anal fissures typically go away on their own within a few weeks. Constipation can readily recur if the reason is left addressed, though. Anal fissure symptoms can remain in some persons for up to six weeks (chronic anal fissures).

In contrast to acute fissures, which have been present for less time, chronic fissures have been present for more than six weeks. Fissures can be treated at home using both conventional drugs and Ayurvedic remedies, whether they are acute or recurrent.

Fissures can be repaired with laser surgery without any discomfort or bleeding. Before the procedure, the patient is given a local anesthetic. Painkillers are frequently used to treat pain following surgery

A rip or incision that is close to the anal opening is known as an anal fissure. With easy at-home treatments and drugs, it frequently gets better in 3–4 weeks. Following laser surgery to fix a fissure, the patient can resume regular activities the subsequent day, and full recovery takes one to two weeks.
