WHAT ARE PILES?

Piles (Mulvyadh/Bavasir), also clinically referred to as Hemorrhoids, occur when the blood vessels near the anal opening swell. Any increased pressure on these blood vessels, for whatever reason, leads to their enlargement and swelling, resulting in the formation of lumps that can occasionally be felt at the anal opening.

WHAT ARE THE SYMPTOMS?

I. MCDPA
(Medication, Constac, Diet, Physiotherapy, Ayurveda therapy)

"An innovation by Dr. Porwal, this conservative approach aims to hinder and reverse the early stages of the disease without resorting to surgical intervention:"

  • Medication : "Treatment includes the use of medications such as creams (Anoac) and stool softeners.
  • Constac: Constac, an herbal preparation formulated by Ayurvedic doctors at Healing Hands & Herbs, is also employed.
  • Diet: Dietary counseling by a qualified nutritionist is provided to address any imbalances and create a diet conducive to healthy bowel habits.
  • Physiotherapy: A pelvic floor physiotherapy is recommended to strengthen muscles in the region.
  • Ayurveda therapy: Ayurveda therapy involves approximately six sessions of enema with medicated herbal oils."

II. LHP
Laser treatment for Piles

  • Laser Hemorrhoidoplasty is the preferred treatment for Grade 2 and early stages of Grade 3 Piles.
  • This procedure is performed under brief general anesthesia and typically lasts around 20 minutes.
  • Using a specialized radially emitting fiber, laser energy is applied through the natural anal opening.
  • The precisely controlled emission of laser energy causes vaporization, resulting in the shrinkage of the hemorrhoidal mass.

III. MIPH
( Minimally Invasive Procedure for Haemorrhoids )

  • Originally conceived by Dr. Antonio Longo in 1994, this procedure has become the preferred treatment for Grade 3 and Grade 4 prolapsed Piles.
  • Typically performed under brief general anesthesia, the procedure lasts around 25-30 minutes.
  • Utilizing a circular stapler, the mucosa responsible for the prolapse of hemorrhoids is circumferentially excised, and the prolapsed hemorrhoids are repositioned back to their normal state.

Potential Consequences of Delayed Treatment for Hemorrhoids

  • Increased Pain and Discomfort : Postponing treatment can result in heightened pain and discomfort, hindering daily activities.
  • Persistent Bleeding : Neglected piles may lead to continuous bleeding, causing anemia or iron deficiency over time.
  • Thrombosis Formation : Untreated piles may develop blood clots, leading to thrombosed hemorrhoids, a painful condition requiring immediate medical attention.
  • Prolapse Exacerbation : Failure to address piles promptly may worsen the prolapse of internal hemorrhoids, making them protrude outside the anus and more challenging to manage
  • Increased Risk of Infection : Open sores and damaged tissue in untreated piles raise the risk of infections, such as cellulitis or abscesses.
  • Chronic Inflammation : Neglected piles can result in chronic inflammation, complicating symptom relief and necessitating more extensive treatment in the long run.
  • Bowel Movement Problems : Unmanaged piles can cause difficulties in passing stools, leading to constipation or a cycle of straining that exacerbates the condition.
  • Impact on Quality of Life : Untreated piles significantly affect daily life, causing emotional distress, impacting work productivity, and limiting social activities due to persistent pain and discomfort.
  • Risk of Complications : Neglected piles may lead to complications like anal fissures, fistulas, or even rectal prolapse, requiring more invasive treatments, potentially leading to surgery.
  • Delayed Diagnosis of Underlying Conditions : Failure to seek timely treatment may postpone the diagnosis of other underlying conditions, such as colorectal cancer, as symptoms may overlap, posing potential harm to overall health."

FAQs

Why do you get piles?

Hemorrhoids occur due to increased pressure on blood vessels in and around the anal region. Chronic constipation stands as the primary cause, with additional factors such as prolonged sitting, obesity, weight-related issues, pregnancy, and lifting heavy weights contributing to the development of piles and hemorrhoids.

When should seek help for Piles?

  • Fever exceeding 100°F
  • Escalating pain, swelling, redness, or discharge
  • Profuse bleeding
  • Constipation lasting more than 3 days

When can I go home?

If your surgery is scheduled as a day care procedure, you can be discharged once the effects of the anesthesia have subsided, you have successfully urinated, and you feel comfortable eating and drinking. As a general anesthetic is administered, it is recommended that a responsible adult accompanies you home and stays with you for 24 hours. Before your discharge, you will receive guidance on post-operative care, pain management, and the use of laxatives.

When will I get back to my routine activities?

You can resume regular physical and sexual activities at your own comfort. It is advisable to avoid anal sex. Typically, you can return to work around the 5th day after surgery, although the timeline may vary depending on the nature of your work.
