Anal fissures

Anal fissures are commonly found in the back part of your anus, in line with the cleft of your buttock. An anal fissure is a common medical condition in which the skin around the opening of the anus opens, leaving a small gap there. The pain caused by this condition usually appears suddenly but you must remember that it can happen gradually as well.

A fissure is a small break in the skin that can occur as a result of poor digestion, chronic irritation, or as a result of smoking. A fissure is also known as an acquired fissure. It occurs at some point in everyone's life, but it may last for less than 6 weeks. In most cases, a fissure will heal itself with no treatment. An acute fissure that lasts longer than 6 weeks requires medical attention such as antibiotic treatment or surgery to remove the split in the skin while a chronic form requires no treatment and heals on its own.

Treatments

1. MCDPA

The MCDPA offers affordable treatment options for anal fissure by providing low cost imaging services and access to board-certified physicians and surgeons when needed.
Medicines - Herbal remedies to lessen localised inflammation
Before bed, take Constac to give your stools more weight and soften them.
Diet: Consume a diet high in fibre to avoid constipation. Increasing fluid and water intake is also beneficial.
Physiotherapy - Pelvic floor physiotherapy aids in lowering anal sphincter spasms.
Ayurvedic therapy employs the use of medicinal herbal oils.

2. Sitz bath

Sitting in warm water that covers your hips and buttocks after a bowel movement helps to reduce post-defecation discomfort. This can be as simple as putting a warm towel between your thighs or using a bidet if that's what is available. If you've evacuated your bowels and then sit in a warm tub or shower, this may reduce the discomfort of your bowel movements. The warmth will help ease any soreness or cramp that is associated with sitting in the same position for an extended period of time.

3. Nitroglycerine

Nitroglycerin or glyceryl trinitrate is a vasodilator that relaxes blood vessels to improve blood flow through the heart, brain, and body. It also helps in reducing spasms of the anal sphincter and thereby helps in improving quality of life. In some cases, this drug may cause certain side effects like headache, light-headedness, etc which limit its role in everyday life.

4. Nifedipine ointment

Nifedipine ointment is used for the treatment of lower urinary tract symptoms, such as incontinence, frequency, and urgency of urination. It relaxes the anal sphincter muscle, which makes it easier for people to pass stool. Nifedipine ointment does not cure conditions caused by certain diseases or disorders, including inflammation caused by infection, kidney disease (nephritis), bladder cancer, or toxic shock syndrome

SURGICAL METHOD

Sr. No. Surgical Method Method
1 Botox injection The anal sphincter is a muscle that can contract and relax. The fissure will not heal if your body fails to contract the sphincter on its own. Botox, a medicine used to relax muscles and tendons, helps achieve this goal by providing an eyelid-like effect on the target muscle. The effects last for three months when using Botox regularly.
2 Lord’s Dilation Dilatation is an age old treatment which provides good sphincter relaxation and relief from pain. Dilatation is a procedure that opens the urinary tract as well as urethra. This provides relief from bladder problems like urinary tract infections, urgency, incontinence and Urinary Tract Obstruction (UTIs).
3 PLIS ( Partial Lateral Internal Sphincterotomy ) Partial Lateral Internal Sphincterotomy (PLIS) is a commonly performed surgical procedure in which a small cut is made in the internal sphincter. This minimally invasive procedure is performed under general anaesthesia, and there are no incisions on the patient's abdominal wall or external genitals.
4 Laser treatment Laser surgery is done with the Leonardo laser introduced for the first time in India at Healing Hands Clinic. The laser is used to perform internal sphincterotomy. It is also known as "Laser sphincterotomy". The procedure involves removing excessive muscle fibers (fibrosis), which are responsible for narrowing the anal canal.
5 BOTOX For Fissure Neurotoxin Botulinum Toxin A is also marketed under the name Botox. It results in a brief, localized, flaccid paralysis of muscle and is commercially available for use in research, cosmetics, and medicine.

What effect does botox have on the fissure?

Botox is a type of medication that helps to temporarily paralyze muscles and reduce the activity levels of certain nerve cells in the body. This can help to reduce blemishes and wrinkles, as well as smooth out your facial features such as crow's feet or frown lines. The results of Botox can last between 3 – 6 months depending on how often you receive treatments.

Causes of anal fissure

A fissure is typically brought on by trauma to the anal canal.

Rare causes include viral infections, Crohn's disease, leukemia, tuberculosis, and sexually transmitted illnesses.
