Overview

Pilonidal sinus disease (PNS) is a chronic, painful condition that affects the area of the skin between the buttocks, near the tailbone. It is typically caused by the formation of an abscess or cyst in that area, which can become infected and cause problems for the patient if left untreated. PNS is most common in young adults, especially those who are overweight, have tight clothing, or don't engage in regular hygiene. It is important to recognize the signs of PNS and to seek medical attention as soon as possible to prevent further complications.

What are the causes of pilonidal sinus disease?

Pilonidal sinus disease is usually caused by a combination of factors. It can be caused by ingrown hairs, loose hairs, or excessive sweating in the area. Other risk factors include sitting for long periods of time, having an overweight build, and having an inactive lifestyle. Additionally, those with thick body hair are more likely to develop PNS due to their higher risk of developing an ingrown hair in the affected area.

The primary cause of PNS is hair or dirt entering the sinus tracts and creating an infection. It can also be caused by ingrown hair, which can be due to long periods of sitting or riding a bike. A person's lifestyle can also contribute to PNS development, as individuals who are frequently exposed to dirt, dust, and sweat may be more likely to develop the condition. In some cases, PNS may be caused by genetic factors, and individuals with a family history of the condition may be more likely to suffer from it.

Signs and symptoms

Your skin may appear dimple-like at first with no obvious symptoms. The depression can quickly develop into an abscess or cyst (a closed sac filled with pus) if it becomes infected.

Infections show the following signs :

  • Having pain while sitting or standing
  • The cyst swells
  • An area of reddened, sore skin
  • The abscess drains pus or blood, causing a foul odour
  • A lesion with hair protruding
  • Multiple sinus tracts or skin holes

There is also a possibility of experiencing a low-grade fever, but this is less common.

Surgery Method

TYPE OF SURGERY OVERVIEW
1. Minor operation to drain pus from sinus - Incision and drainage
  • A general anaesthetic or local anaesthetic may be used.
  • Regular (daily) changes of clothing
  • Takes 4 to 6 weeks to recuperate.
2. Operation to remove sinuses (wound left open) - Extensive removal and open healing
  • Hospital stays under general anaesthesia.
  • Regular changes of clothing
  • 6 to 12 weeks of recuperation time
3. Operation to remove sinuses (wound closed) - Excision, wound healing, and frequently flattening of the buttocks groove
  • Hospital stays under general anaesthesia
  • Compared to extensive excision and open healing, which has a faster recovery time.
  • Stitches removed in 10 days following surgery.
  • Higher risk of infection
4. Laser Pilonidoplasty (LPP)
  • Patient is released after 24 hours
  • Since a wide excision typically requires 6 to 8 weeks for wound healing,
  • Recovery is noticeably quicker.
  • Recurrence rates are quite low.

Plastic surgery can be used when a large area needs to be treated. It is necessary to reconstruct the surrounding skin after removing the sinuses.

Frequently Asked Questions (FAQ's)

Although the cyst is not serious, it can become infected, so it should be treated. As a result of infection, a pilonidal cyst develops a sinus abscess, which eventually drains pus. There is pain, a foul smell, and drainage from the abscess. There is no danger associated with this condition.

Symptoms of swelling and pus leakage can be temporarily relieved, but they usually return in a short time. It is rare for pilonidal sinus infections to resolve on their own.

The pilonidal sinus is a small pocket on the skin just above the hip bone. It's basically an enlarged hair follicle that fills with fluid, creating a bump. The pilonidal sinus won't go away by itself, and it can become quite painful if not treated properly.

The condition can be caused by several factors like hormone imbalance, bacterial infection and eczema etc. In some cases, pilonidal sinus could be linked with certain other diseases as well

Laser Pilonidoplasty (LPP)
Advantages -

  • After 24 hours of treatment, the patient is discharged and can return to work on the 5th day.
  • The wound heals in about 6-8 weeks and recovery is much faster than with conventional wide excisions.
  • Negligible Recurrences have been reported.
