What Is Hernia

A hernia occurs when an internal organ or other body part extends through the wall of muscle or tissue that normally contains it. The majority of hernias occur in the abdominal cavity, between the chest and hips.

What are the symptoms of hernia?

When lying down, a hernia in the abdomen or groin can cause a visible lump or bulge that can be pushed back in or disappear. Laughing, crying, coughing, straining during bowel movements, or engaging in physical activity may cause the lump to reappear after it has been pushed in. A hernia can cause the following symptoms :

  • Swelling or bulging in the groin or scrotum
  • Pain at the bulge site has become more severe.
  • Lifting causes pain.
  • The size of the bulge grows larger over time.
  • A dull aching feeling.
  • A feeling of fullness or signs of bowel obstruction

How common are hernias?

Of all hernias that develop :

  • 75 to 80% are inguinal or femoral.
  • Only 2% are incisional or ventral.
  • 3 to 10% are umbilical, affecting 10 to 20% of newborns; most close by themselves by the age of 5 years.
  • Other types range from 1% to 3%.

Surgery is the only permanent treatment for hernias. With many hernias remaining asymptomatic for long periods, an obvious question that may occur is, 'What if I leave my hernia untreated?' According to the medical community, 'all hernias should be fixed unless severe pre-existing medical conditions make surgery unsafe.' Many patients, particularly those with a smaller, asymptomatic hernia who are afraid of undergoing surgery, choose not to have surgery. It should be noted, however, that:

  • Asymptomatic hernias typically cause pain and discomfort over time. Additionally, mild existing symptoms usually worsen over time.
  • If you keep delaying surgical treatment, the hernia will most likely grow larger over time, making future repair technically more difficult.
  • The sooner the hernia is repaired, the smaller the hernia and there is less trauma from surgery. Surgery for a smaller hernia usually results in a faster recovery.
  • The threat of strangulation is always present. It is more common with smaller hernias, and you certainly don't want it while on vacation in a remote location or before an important event like a wedding.

1. 3d mesh repair - an excellent treatment for hernia -

This is the most advanced and preferred method for repairing the majority of hernias. It is a straightforward and promising method that covers the hernia defect from three angles: above, below, and in the centre.

A mesh is a supporting structure that is used to strengthen the hernia repair. It is used to conceal the flaw and relieve tension during the repair. Otherwise, tension can cause the hernia to recur.

The mesh is constructed from a synthetic material such as polypropylene or polyester. The mesh could also be a biologic graft derived from animal tissue. Because biologic grafts are resistant to infections, they are frequently used when there is a high risk of infection.

2. laparoscopic repair -

Laparoscopic surgery involves making small incisions in the abdominal wall through which instruments are inserted into the abdominal cavity. The abdomen is inflated with air, and the repair is guided by a camera. The contents of the hernia are returned to the abdominal cavity, and a mesh is secured in place to cover the hernia defect from the inside. The mesh aids in preventing its recurrence. The advantages of laparoscopic surgery include faster recovery, less pain, and a lower risk of infection.

3. open repair -

The traditional approach to hernia repair surgery is open repair, which involves making an incision over the hernia. The contents of the sac are returned to the abdominal cavity, and the sac is removed. A mesh is frequently used to cover the area of the hernia defect, depending on factors such as the location and size of the hernia. Otherwise, stitches are used to close the defect.

The open approach is commonly used for very large hernias or those that are complicated by bowel obstruction, infection, incarceration, strangulation, or recurrences.

  • INGUINAL HERNIA
    This is the most common type of hernia in the groin area at the top of the inner thigh. It is more common in men and is linked to getting older and repetitive abdominal stress. An inguinal hernia, or its repair, should not be taken lightly because, the blood vessels that supply the testicles pass through the areas where this hernia occurs, neglect in either situation can impair blood supply to the testicles. The testicular tissue may die due to a lack of blood supply, necessitating removal.

  • FEMORAL HERNIA
    This is more common in women and occurs in the groin area, just above the line that separates the abdomen and the legs. A femoral hernia should not be ignored because it increases the risk of strangulation, which is a medical emergency. Nearly half of all femoral hernias are discovered as an emergency. As a result, femoral hernias should be repaired as soon as possible, before complications arise.

  • UMBILICAL HERNIA
    It happens when fat or bowel material stretches through the abdomen near the belly button. It occurs in babies when the opening in the abdomen by which the umbilical cord passes fails to seal properly after birth. It is also found in adults as a result of a repeated abdominal strain.

  • EPIGASTRIC HERNIA
    It appears in the midline of the abdomen, between the naval and the lower portion of the breastbone (sternum). Because it emerges between the two rectus muscles of the abdomen that meet in the midline, this hernia always occurs in the midline. Even though the lump may appear to be off the midline at times, the opening is always in the midline. It can also be found in infants due to congenital abdominal wall weakness in the midline.

  • INCISIONAL HERNIA
    It happens at the site of a previous abdominal surgery for another reason, during which the abdominal muscles were cut open to allow the surgeon to enter the abdominal cavity and perform surgery. Despite the fact that the muscle is sutured (stitched) during closing, it becomes a relative weakness, potentially allowing abdominal organs to herniate through the incision line.
