What is Constipation?

Constipation is characterized by infrequent bowel movements, leading to the passage of hard, dry, and lumpy stools that are challenging or painful to expel. It is defined as having fewer than three bowel movements per week. While occasional constipation is more prevalent, some individuals experience chronic constipation, which can significantly impede daily activities. Those with chronic constipation may also engage in excessive straining during bowel movements. Occasional constipation is a common experience, usually resolving without serious implications after a short duration. However, if severe pain, blood in stools, or constipation persists for more than three weeks, it is advisable to consult with a doctor.

Diagnosis

  • Medical and Family History Assessment :
    - Doctors inquire about your medical history and any digestive tract surgeries, as well as your family's history of persistent constipation.
  • Physical Examination :
    - A physical exam is conducted to assess vital signs such as heart rate, blood pressure, and temperature.
    - The use of a stethoscope helps listen to abdominal sounds.
  • Rectal Examination :
    - A rectal examination is performed as part of the assessment.
  • Endoscopic Examination :
    - The doctor may conduct an endoscopic examination to visualize the interior of your anus, rectum, and colon.
  • Additional Bowel Function Examinations :
    - Other bowel function examinations may be recommended, including :
    - Defaecography: Evaluates how effectively you can retain and expel stool.
    - Anorectal Manometry: Assesses the functionality of your sphincter muscles.

Causes

  • Irritable Bowel Syndrome (IBS) : Individuals with functional intestinal difficulties, such as Irritable Bowel Syndrome (IBS), face a higher risk of constipation compared to those without IBS.
  • Aging : The prevalence of constipation tends to increase with age. As individuals age, the digestive process slows down, and reduced mobility can contribute to constipation.
  • Dietary Changes : Constipation may result from dietary factors, particularly a lack of fiber in the diet. Regular consumption of non-home-cooked foods can disrupt bowel movements. Additionally, a high intake of dairy products is associated with an increased risk of constipation.
  • Dehydration : Insufficient water intake can contribute to constipation. When the body is dehydrated, the large intestine absorbs water from waste, resulting in hard and difficult-to-pass stools.
  • Ignoring the Urge to Defecate : Resisting the urge to poop and delaying bathroom visits can lead to constipation. It is essential to respond promptly when the need arises.
  • Stress and Irregular Sleep Patterns : Both stress and irregular sleep patterns can impact bowel regularity, potentially contributing to constipation.
  • Overuse of Laxatives : Excessive reliance on laxatives can lead to constipation over time, creating a dependency on these substances for bowel movements.
  • Hormonal Problems (e.g., Hypothyroidism) : Hormonal imbalances, such as hypothyroidism, can affect digestive processes and contribute to the development of constipation.

Treatment for constipation

In the majority of constipation cases, medical intervention is seldom necessary, and lifestyle adjustments and dietary modifications often suffice to provide relief. Conservative measures, such as altering toilet posture, incorporating pre and probiotics, engaging in pelvic muscle training, maintaining regular exercise, and using stool softeners, are effective in many instances.

When conservative treatments prove ineffective, surgical intervention may be considered, contingent on the underlying cause of constipation. Major subgroups contributing to constipation include normal transit constipation, slow transit constipation, and pelvic floor dysfunction. Surgery may be beneficial in cases involving blockages, anal fissures, or rectal prolapse.

In situations where dietary changes and laxatives offer no relief, Obstructed Defaecation Syndrome (ODS) may be present. When ODS is associated with structural anomalies such as prolapse, STAPLED TRANS ANAL RECTAL RESECTION (STARR), introduced by the renowned colorectal surgeon Dr. Antonio Longo, has shown beneficial results. The performance of STARR significantly improves constipation in individuals with this condition.
