ANAL FISSURE

Acute anal fissures are typically fresh tears, much like a paper cut. The fissure can appear anywhere along the anal canal, usually near the anus or in the midline. These fissures typically cause intense pain and discomfort, as well as bleeding and itching. Treatment of acute anal fissures usually involves topical ointments or creams to reduce inflammation and promote healing

Causes

The lining of the anus or anal canal, the final section of the large intestine, is most frequently the source of anal fissures. The majority of occurrences happen in constipated patients when a particularly large or hard poop breaks the lining of the anal canal. Persistent diarrhoea is another factor that could contribute to anal fissures.

What symptoms indicate an anal fissure?

Anal fissure symptoms include:

  • Having bowel movements that hurt.
  • Discomfort following bowel movements that may continue for several hours.
  • After a bowel movement, there is bright red blood on the toilet paper or stool.
  • An apparent skin split near the anus.
  • A little bump or mole on the skin next to the anal fissure.

Surgical treatments

Surgery If alternative therapies have failed, surgery could be advised. With over 90% of patients reporting positive long-term effects, it is usually regarded as the most successful treatment for anal fissures. There is a slight possibility of difficulties, though.

Anal fissures can be treated surgically using a variety of different methods.

  • PLIS (Partial Lateral Internal Sphincterotomy) : It is a surgical procedure that is used to treat anal fissures. This procedure involves making a small incision in the anal sphincter muscle in order to relax the muscle and reduce spasms, which can help to improve the healing of the fissure. Local anesthesia and sedation are used during the procedure. After PLIS, patients may experience some discomfort for a few days, but this can usually be managed with pain relief medications.
  • Lateral sphincterotomy In order to lessen the stress in your anal canal, a lateral sphincterotomy is making a little cut in the ring of muscle encircling the sphincter. This promotes healing of the anal fissure and lowers your risk of getting any further fissures.
  • Botulinum toxin injection A relatively recent treatment for anal fissures is botulinum toxin. When previous medications haven't worked, this one is typically utilized. A potent poison, botulinum toxin is safe to employ in modest quantities. An injection of the poison can be used to paralyze your sphincter muscle if you have an anal fissure. This should stop the muscle from spasming, hence minimizing pain and promoting fissure healing.
  • MCDPA
    • Herbal treatments for medicines to reduce localized inflammation
    • Take Constac before bed to soften and give your stools more weight.
    • Diet: To prevent constipation, eat a diet rich in fiber. Ingesting more liquids and water is also advantageous.
    • Anal sphincter spasms can be reduced by physiotherapy, specifically pelvic floor physiotherapy.
    • Therapeutic herbal oils are used in Ayurvedic therapy.

Home Remedies

Acute fissures can be treated at home with a Sitz bath, application of olive, coconut, or ghee oil, consumption of 1-2 tablespoons of apple cider vinegar, and a diet high in fiber to keep the stools soft. An acute fissure frequently heals by itself. However, it's recommended to see a proctologist if it doesn't heal in 4-5 days or if you suffer from a persistent fissure.
