What is a pilonidal sinus?

Pilonidal sinuses are abnormal openings just under the skin that appears when hair grows into it. It's also generally known as "the valley of pain" or "gluteal cleft cyst." The symptoms of pilonidal sinus include chronic pain, a thickened area of skin, and/or pus. The pilonidal cyst can form on any part of your body — but is most common around your buttock or hips. Sometimes, if you don't do anything to treat it and leave it alone, it will get bigger and eventually cause infection within the sac

What causes pilonidal sinus?

Pilonidal sinuses are a type of swelling on the skin caused by infection and inflammation. They develop when hair follicles become blocked with debris, leading to inflammation and collections of pus that drains out under the skin's surface. Pilonidal cysts in males are called inguinal (meaning under the groin) pilonidal cysts; in females, they are known as inguinal pilonidal cysts.

Most pilonidal cysts are brought on by loose hairs that pierce the skin. Hair can be forced into the skin by friction and pressure from rubbing, tight clothing, cycling, or extended periods of sitting.

How to treat it?

TYPE OF SURGERY OVERVIEW
1. Minor operation to drain pus from sinus - Incision and drainage
  • A general anaesthetic or local anaesthetic may be used.
  • Regular (daily) changes of clothing
  • Takes 4 to 6 weeks to recuperate.
2. Operation to remove sinuses (wound left open) - Extensive removal and open healing
  • Hospital stays under general anaesthesia.
  • Regular changes of clothing
  • 6 to 12 weeks of recuperation time
3. Operation to remove sinuses (wound closed) - Excision, wound healing, and frequently flattening of the buttocks groove
  • Hospital stays under general anaesthesia
  • Compared to extensive excision and open healing, which has a faster recovery time.
  • Stitches removed in 10 days following surgery.
  • Higher risk of infection
4. Laser Pilonidoplasty (LPP)
  • Patient is released after 24 hours
  • Since a wide excision typically requires 6 to 8 weeks for wound healing,
  • Recovery is noticeably quicker.
  • Recurrence rates are quite low.

What consequences might pilonidal sinus disease cause?

A variety of issues could result from PNS. Among these are wound infection and a PNS recurrence even following surgery.

Symptoms of an infected wound include :

  • A wound site with extreme pain,
  • Inflamed, swollen skin,
  • A temperature of 100.4°f or higher,
  • Blood, pus, and an offensive odour

Frequently Asked Questions (FAQ's)

On the top of your buttocks, a painful, sensitive, swollen, and red region will develop. You can feel more discomfort when you sit down because pus might start to ooze from the cyst. The pilonidal sinus is covered in hair as well.

In comparison to other procedures, such as those performed to treat fistulas and fissures, the pain associated with pilonidal sinus surgery is frequently less. You won't feel any discomfort during or after the procedure because of the local anaesthetic.

Despite the cyst's lack of seriousness, it should nevertheless be treated because it could become infected. A sinus abscess forms in a pilonidal cyst as a result of infection, and it eventually drains pus. The abscess is painful, has a bad odour, and is draining. There is no risk related to this situation.

Swelling and pus-leaking symptoms can be momentarily eased, but they typically recur soon after. Pilonidal sinus infections rarely go away on their own.

Pilonidoplasty with laser (Lpp) is the most effective for pilonidal sinus.
Benefits -

  • Following a 24-hour period of therapy, the patient is released and may resume work on the fifth day.
  • Recovery from the wound takes 6 to 8 weeks on average, which is substantially faster than with traditional wide excisions.
  • There have been very few recorded recurrences.
