What is anal fistula ?

An anal fistula is a medical condition characterized by the formation of an infected passageway between the skin and the muscular opening located at the end of the digestive tract, known as the anus.

Typically, anal fistulas arise from an infection originating in the anal gland. This infection leads to the development of an abscess, which may either discharge spontaneously or be surgically drained through the skin adjacent to the anus. Subsequently, the fistula progresses beneath the skin and establishes a connection with the infected gland.

Causes Of Fistula

The primary cause of almost all anal fistulas is an anorectal abscess. An anorectal abscess occurs due to an infection in one of the anal glands, which rapidly progresses into the formation of a pus-filled collection known as an abscess. When the abscess fails to heal completely, with or without treatment, it can lead to the development of an anal fistula.

Other factors that can contribute to the occurrence of anal fistulas include :

  • Inflammatory conditions affecting the intestines, such as Crohn's disease and diverticulitis.
  • Anorectal cancer.
  • Infections caused by tuberculosis, HIV, or sexually transmitted diseases.
  • Iatrogenic causes, resulting from previous surgical interventions in the area.

Treatment Options for Fistula

FiLaC (Fistula-tract Laser Closure) :

  • The procedure typically lasts around 30-40 minutes.
  • The objective is to delicately remove the fistula tract while ensuring the sphincter muscles remain unharmed.

Distal Laser Proximal Ligation (DLPL) :

  • First, the abscess pit and the internal opening are excised.
  • The closure of the distal tract takes this into consideration.
  • The Leonardo Laser is then employed to close the proximal end of the fistula tract.
  • he laser emits radiation radially through a fiber, effectively sealing the tract completely.

VAAFT (Video Assisted Anal Fistula Treatment) :

  • This approach is used for the surgical treatment of complex fistulas.
  • It involves the utilization of a Fistuloscope.
  • The procedure consists of two stages: the Diagnostic Stage and the Treatment Stage.

LIFT (Ligation of Intersphincteric Fistula Tract) :

  • LIFT is typically performed for complex or deep fistulas.
  • The fistula is accessed between the sphincter muscles, avoiding the need to cut them.
  • The success rate of LIFT is approximately 70%, with a healing impairment observed in 20-30% of cases.

Fistula Plug

  • A fistula plug is a fully synthetic bio-absorbable scaffold used in this treatment method.
  • It does not involve cutting or creating an operative wound, resulting in less pain and faster recovery.
  • The sphincter muscle remains undamaged during the procedure.

Fistulectomy

  • Laser is utilized as an adjunct to Fistulectomy to minimize the risk of sphincter muscle damage.

Ksharsutra

  • This non-invasive technique does not involve any cuts or stitches.
  • It has a fairly good success rate and does not cause damage to the sphincter muscle.

Frequently Asked Questions (FAQ's)

It is advisable to seek help for a fistula in the following situations:

  • Fever exceeding 101 degrees Fahrenheit (38.3 degrees Celsius).
  • Persistent pain that does not subside with the use of medications.
  • Experiencing bleeding during bowel movements.
  • Constant feelings of nausea or recurrent vomiting.

If you encounter any of these symptoms related to a fistula, it is recommended to seek prompt medical attention.

Typically, it takes approximately 6-8 weeks for a Fistula to heal completely. Following the surgical procedure, it is important to allow yourself to rest completely for the first 24 hours. Afterward, you can gradually resume your daily activities. In general, most individuals are able to return to their normal routines within 2-3 days.

To aid in the healing process, it is recommended to:

  • Get ample rest to allow your body to recover.
  • Take Sitz baths three times a day, particularly after a bowel movement. This can help in soothing the affected area.
  • Maintain good hygiene by keeping the area clean and dry.
  • Follow a diet that is rich in fiber. This can facilitate smooth bowel movements and prevent strain.

By implementing these measures, you can support the healing of the affected area and promote a faster recovery.

No specific diet is required. It is advisable to maintain a healthy eating routine that includes a good amount of fiber and protein both before and after your surgery.

Shortly after the procedure, you can begin drinking water and resume eating when you feel hungry. After the surgery, you will be able to get out of bed and move around. It is common to experience some pain after the surgery, but this can be effectively managed with the use of painkillers.
