An anal fissure or fissure-in-ano is a tear or cut in the skin of the anal canal. The condition, which can emerge at any age, affects both men and women equally ( infants too ). Anal fissures are frequently found along the buttock cleft in the back of your anus.

Fissures that have been persistent for less than six weeks are considered acute. Any fissure older than six weeks is considered a chronic fissure. It can be challenging to feel the Sentinel Pile, which mimics a skin tag and may exist in the Chronic fissure.

Treatment

Medication - Medications are frequently used to treat acute fissures (those that are younger than 6 weeks). Even Chronic fissures go through a trial run of patient care. The aim of the therapy is to stop the cycle of anal sphincter spasms and anal skin tears.

1. MCDPA

  • Herbal treatments for medicines to reduce localized inflammation
  • Take Constac before bed to soften and give your stools more weight.
  • Diet: To prevent constipation, eat a diet rich in fiber. Ingesting more liquids and water is also advantageous.
  • Anal sphincter spasms can be reduced by physiotherapy, specifically pelvic floor physiotherapy.
  • Therapeutic herbal oils are used in Ayurvedic therapy.

2. A seat bath

Relaxing in warm water that surrounds your hips and buttocks after a bowel movement will help relieve any soreness.

3. Nitroglycerin

When used topically, it helps to decrease the spasm of the anal sphincter. Its unfavorable side effects, such as headaches and lightheadedness, however, restrict its applicability.

4. Nifedipine cream

This technique also relaxes the anal sphincter.

Surgical methods

Surgical Method Method
Botox injection By momentarily numbing the muscles below the anal sphincter, Botox injections can help lessen or even eliminate fissures, while they are not a cure. This stops feces from passing unintentionally through the anus.
Lord’s Dilation A speculum is inserted into a patient's rectum at various angles during the lord's dilatation (or lord's), which entails enlarging the anus that is already open. This operation is performed to provide the patient extra room and to ease constipation or hemorrhoid pain.
PLIS ( Partial Lateral Internal Sphincterotomy ) PLIS ( Partial Lateral Internal Sphincterotomy ) for anal fissure is performed under local anesthesia and sedation. The patient is positioned in a side-lying position on the operating table with hips flexed toward the operating table, knees bent and feet elevated on a padded stirrup. A small incision is made over the lateral aspect of the buttocks so that it can be used as an outlet to drain any fecal matter present at the time of surgery.
Laser treatment This is done using the Leonardo laser, which was first employed at the Healing Hands Clinic in India. A laser is used to perform internal sphincterotomy. This procedure results in almost no blood loss, and the surgeon has exceptional control over the operative site. The chronic fibrotic scar is also removed with the laser. This prevents any long-term discomfort and hastens the healing process.
BOTOX For Fissure Neurotoxin Botulinum Toxin A is also marketed under the name Botox. It results in a brief, localized, flaccid paralysis of muscle and is commercially available for use in research, cosmetics, and medicine.

What effect does botox have on the fissure?

Botox is an injectable medication that is used to smooth out lines on the face and minimize wrinkles. A naturally occurring protein called botulinum toxin, also referred to as botox, controls muscular action by preventing impulses from moving back and forth between nerve cells. This can result in muscle weakness when it is injected and soft, young faces.

How is Botox administered?

You will be momentarily sedated to decrease the discomfort because the anal region is so delicate. Local anesthetics can occasionally be utilized to numb, negating the need for sedation.

After the area has been cleansed, Botulinum Toxin is injected into the sphincter muscle surrounding the anus.

Out of the 20 to 30 minutes that the procedure as a whole takes, the actual injection duration is only approximately 5 minutes. You can go home as soon as you're at ease.

What happens after the process?

Painkillers may be administered to lessen any discomfort experienced after the injection. The benefits of Botox began to appear over the following 72 hours and peaked at 4 weeks, lasting for 3-6 months ( individual variation ). Maintaining soft stools requires a diet high in fiber and the use of stool softeners as recommended.

Possible side effects include bleeding, transient incontinence (inability to control bowel motions), an allergic reaction to Botox, muscle weakness, and postural hypotension (found in a small percentage of patients).

Conclusion

Because it offers a non-surgical option for treating chronic anal fissures, Botox is so well-liked. The majority of folks have at least some pain relief from it. It works in around 60–70% of circumstances.

After six months, it is easily repeatable, which is an added advantage. It's possible that a very small fraction of people won't respond to it, in which case it would be important to look into other treatment options.

Is the surgery for fissures necessary?

Chronic fissures are ones that have been present for longer than six weeks as opposed to acute fissures, which have been present for less time. Both conventional medications and Ayurvedic treatments can be used to treat fissures at home, whether they are acute or persistent.

Frequently Asked Questions (FAQ's)

Laser surgery can heal fissures without causing pain or bleeding. The patient receives a local anesthetic before the surgery. Painkillers are typically used to relieve post-operative pain.

An anal fissure is a tear or incision that is near the anal orifice. It often improves in 3–4 weeks with simple home remedies and medications. The patient can resume normal activities the next day following laser surgery to treat a fissure, and full healing takes one to two weeks

For fissures, the best procedure is laser sphincterotomy. The anal fissure was treated with laser therapy using the Leonardo Laser

Constipation-causing foods should be avoided. You should consume extra water and include fiber-rich food in your diet to prevent constipation. Since constipation is the leading cause of fissures.
