Pilonidal Sinus

A persistent, excruciating ailment known as pilonidal sinus disease (PNS) affects the skin between the buttocks, close to the tailbone. It is often brought on by the development of an abscess or cyst in that location, which, if left untreated, can grow infectious and pose issues for the patient. Young individuals are most susceptible to PNS, particularly those who are overweight, wear restrictive clothing, or don't practise good hygiene. To avoid future difficulties, it's critical to detect the PNS symptoms and get medical help as soon as you can.

What triggers a pilonidal sinus condition?

Pilonidal sinus illness is typically brought on by a number of different circumstances. Ingrown hairs, loose hairs, or excessive perspiration in the area are possible causes. Long durations of inactivity, having an overweight build, and sitting too much are other risk factors. In addition, PNS is more common in people with thick body hair because ingrown hairs are more prone to form in the affected area.

Hair or debris getting into the sinus passages and causing an infection is the main cause of PNS. It may also be brought on by an ingrown hair, which may develop as a result of prolonged sitting or riding a bike.

The way a person lives can also affect the development of PNS, since those who are constantly exposed to perspiration, dirt, and other irritants may be more prone to the condition. PNS may occasionally be inherited, and those who have a family history of the disorder may be more likely to experience it.

Symptoms and signs

Your skin may first look bumpy but have no evident symptoms. If the depression gets infected, it can soon turn into an abscess or cyst (a closed sac filled with pus).

Infections manifest as the following :

  • Having discomfort whether standing or sitting
  • The cyst enlarges
  • A spot of irritated, inflamed skin
  • The abscess leaks pus or blood, which produces a bad stench.
  • A growth with hair sticking out
  • Many skin pores or sinuses
  • A low-grade fever is another potential side effect, albeit it is less typical.

Surgical Procedure

TYPE OF SURGERY OVERVIEW
1. Minor operation to drain pus from sinus - Incision and drainage
  • A general anaesthetic or local anaesthetic may be used.
  • Regular (daily) changes of clothing
  • Takes 4 to 6 weeks to recuperate.
2. Operation to remove sinuses (wound left open) - Extensive removal and open healing
  • Hospital stays under general anaesthesia.
  • Regular changes of clothing
  • 6 to 12 weeks of recuperation time
3. Operation to remove sinuses (wound closed) - Excision, wound healing, and frequently flattening of the buttocks groove
  • Hospital stays under general anaesthesia
  • Compared to extensive excision and open healing, which has a faster recovery time.
  • Stitches removed in 10 days following surgery.
  • Higher risk of infection
4. Laser Pilonidoplasty (LPP)
  • Patient is released after 24 hours
  • Since a wide excision typically requires 6 to 8 weeks for wound healing,
  • Recovery is noticeably quicker.
  • Recurrence rates are quite low.

When a wide area needs to be treated, plastic surgery may be employed. After the sinuses are removed, the surrounding skin must be rebuilt.

Frequently Asked Questions (FAQ's)

Despite the cyst's lack of seriousness, it should nevertheless be treated because it could become infected. A sinus abscess forms in a pilonidal cyst as a result of infection, and it eventually drains pus. The abscess is painful, has a bad odour, and is draining. There is no risk related to this situation.

Swelling and pus leaking symptoms can be momentarily eased, but they typically recur soon after. Pilonidal sinus infections rarely go away on their own.

A little skin pocket located right above the hip bone is known as the pilonidal sinus. In essence, it is a fluid-filled, swollen hair follicle that develops a hump. If not appropriately treated, the pilonidal sinus can become highly painful and won't go away on its own.

Numerous factors, including hormonal imbalance, bacterial infection, and eczema, among others, might contribute to the illness. Pilonidal sinus may occasionally be related to other conditions.
