An anal fissure, also known as Fissure-in-ano, is a small tear or cut in the lining of the anal canal. This condition can affect men and women of any age, including infants. Anal fissures are commonly located at the back of the anus, in alignment with the cleft of the buttocks.
Fissures are categorized based on their duration :
Prompt treatment is essential to alleviate discomfort and prevent complications.
Medicines
For acute fissures (less than six weeks old), medications are the first line of treatment. Chronic fissures are managed through initial conservative measures to break the cycle of sphincter spasms and anal skin tears.
MCDPA Approach
2. Sitz Bath
A warm water soak after bowel movements helps to relax the anal muscles, reduce soreness, and enhance comfort.
Topical Treatments
At Healing Hands Clinic, we offer a well-rounded approach, blending traditional methods with advanced treatments to ensure rapid healing and long-lasting relief from anal fissures.
Surgical Method | Method |
---|---|
Botox injection | By momentarily numbing the muscles below the anal sphincter, Botox injections can help lessen or even eliminate fissures, while they are not a cure. This stops feces from passing unintentionally through the anus. |
Lord’s Dilation | A speculum is inserted into a patient's rectum at various angles during the lord's dilatation (or lord's), which entails enlarging the anus that is already open. This operation is performed to provide the patient extra room and to ease constipation or hemorrhoid pain. |
PLIS ( Partial Lateral Internal Sphincterotomy ) | PLIS ( Partial Lateral Internal Sphincterotomy ) for anal fissure is performed under local anesthesia and sedation. The patient is positioned in a side-lying position on the operating table with hips flexed toward the operating table, knees bent and feet elevated on a padded stirrup. A small incision is made over the lateral aspect of the buttocks so that it can be used as an outlet to drain any fecal matter present at the time of surgery. |
Laser treatment | This is done using the Leonardo laser, which was first employed at the Healing Hands Clinic in India. A laser is used to perform internal sphincterotomy. This procedure results in almost no blood loss, and the surgeon has exceptional control over the operative site. The chronic fibrotic scar is also removed with the laser. This prevents any long-term discomfort and hastens the healing process. |
BOTOX For Fissure | Neurotoxin Botulinum Toxin A is also marketed under the name Botox. It results in a brief, localized, flaccid paralysis of muscle and is commercially available for use in research, cosmetics, and medicine. |
Botox is an injectable treatment commonly used to reduce wrinkles and smooth facial lines. It contains botulinum toxin, a naturally occurring protein that works by interfering with the signals between nerves and muscles. This causes temporary muscle relaxation in the injected area, leading to softer facial expressions and muscle weakness. When used for anal fissures, Botox is injected into the anal sphincter to reduce spasms, ease pain, and promote healing by relaxing the muscle.
Due to the sensitivity of the anal area, mild sedation is usually given to ensure comfort during the procedure. In some cases, local anesthesia may be sufficient to numb the area instead of sedation.
After cleaning the site thoroughly, botulinum toxin is carefully injected into the sphincter muscle surrounding the anus. While the entire procedure takes about 20 to 30 minutes, the injection itself is completed within approximately 5 minutes. You can head home shortly after the procedure once you feel comfortable.
In order to ease any discomfort following the injection, painkillers may be given. The next 72 hours saw a gradual onset of Botox's effects, which peak at 4 weeks and last for 3-6 months ( individual variation ). A diet high in fiber and the use of stool softeners as directed is crucial for maintaining soft stools.
Bleeding, temporary inability to regulate bowel movements or flatulence (incontinence), an allergic reaction to Botox, muscle weakness, and postural hypotension are possible adverse effects (found in a small percentage of patients)
Botox has gained popularity as a non-surgical option for treating chronic anal fissures, offering pain relief for most patients. Its effectiveness is observed in approximately 60–70% of cases, and it can be easily repeated after six months if needed. For a small percentage of individuals where Botox is less effective, alternative treatment options can be explored.

Anal fissures are typically caused by trauma to the anal canal, often due to passing large, hard stools or straining during bowel movements. Chronic constipation, diarrhea, and conditions like Crohn's disease can also contribute to fissure formation.
To prevent fissures, maintain a high-fiber diet, drink plenty of water, and exercise regularly to avoid constipation. Using a stool softener during periods of hard stools can also help reduce the risk.
Acute fissures (present for less than six weeks) often heal on their own or with conservative treatments like diet changes, sitz baths, and ointments. Chronic fissures (lasting more than six weeks) may require advanced treatment, including Botox or surgery.
No, surgery is not the only solution. Treatments like Botox injections, which relax the sphincter muscle, and other non-invasive therapies are effective options for chronic fissures. Surgery is considered only when other treatments fail.
