Pilonidal sinus is a painful condition that affects the skin and tissue in the buttocks area, particularly around the tailbone. It primarily occurs in young adults, especially those with deep skin folds. The condition results from the blockage of hair follicles, which leads to the accumulation of pus and bacteria. It often starts as a small cyst or bump, but without treatment, it can become infected and cause significant pain. Pilonidal sinus is commonly treated with surgical intervention to prevent recurring infections and relieve discomfort.
Managing Pilonidal sinus involves a intricate procedure wherein the cyst and the underlying sinus are surgically removed. The objective of this treatment is to minimize infection and attain a sustained recovery. In some cases, the initial procedure may prove insufficient, necessitating additional interventions. Depending on the specific cause, the physician may prescribe medications to alleviate swelling and facilitate the healing process.
Dormant Pilonidal sinuses, which are not infected, typically do not require treatment. However, precautions should be observed during this latent phase to mitigate the risk of infection and consequently, the need for surgery.
It is crucial to observe the following safety measures :
A Pilonidal sinus that is infected can lead to pain and discharge, potentially necessitating surgical intervention.

A pilonidal sinus — called Nali Ka Ghav (नाली का घाव) in Hindi — is a small infected tunnel at the top of the buttocks cleft containing hair, skin debris, and bacteria. It forms when loose hairs penetrate the skin, causing a foreign body reaction that leads to an infected tract. Simple drainage only treats infection temporarily. Without complete surgical removal of the sinus and all its openings, recurrence is common. Permanent laser hair removal of the natal cleft is essential to prevent recurrence.
Healing Hands Clinic Lucknow is nationally recognised for pilonidal sinus treatment, led by Dr. Rachit Raj — MBBS from KGMU Lucknow, Gold Medallist in MS General Surgery, FMAS, MCLS, and ATLS Instructor. Patients from across Uttar Pradesh and neighbouring states visit HHC Lucknow, making it a premier destination for pilonidal care in North-Central India.
Available treatments include:
LPP uses a radial laser fiber to destroy the sinus lining, debride contents, and seal the tract through its external opening — no large incision needed. Ideal for uncomplicated, non-recurrent pilonidal sinuses with few tracts. Advantages: minimal scarring, daycare discharge, and return to activities in 3–5 days. Not suitable for complex or recurrent cases.
The Limberg Flap is a rhomboid skin flap rotated to cover the defect after wide excision, eliminating the deep natal cleft that traps hair. Recommended for recurrent disease, wide defects, or when primary closure causes wound tension. Recurrence rates are very low (2–3%).
Loose or ingrown hairs cause pilonidal sinus formation and recurrence. Permanent laser hair removal after surgery reduces hair regrowth and prevents recurrence. Shaving or waxing is temporary and may worsen hair penetration.
